Services Description

Our Registered Nursing Services provide professional, patient-centered care for individuals requiring clinical support in the home or clinic setting. Care is delivered by licensed Registered Nurses (RNs) and Registered Practical Nurses (RPNs), matched appropriately to each patient’s condition, stability, and care requirements.

Services are delivered in accordance with physician orders, individualized care plans, and Ontario nursing standards of practice. Our goal is to ensure safe, effective, and coordinated care that supports health, comfort, and quality of life.

Registered Nurse (RN) Services

Registered Nurses provide advanced clinical care for patients with complex, unstable, or high-risk needs. Typical RN services include:

Comprehensive health and nursing assessments
Development, review, and modification of individualized care plans
Complex wound care and advanced wound therapies, including NPWT
IV therapy, infusions, and central line care
Complex medication administration and medication reconciliation

Chronic disease management for high-risk or unstable patients
Palliative and end-of-life care focused on comfort and symptom management
Clinical supervision and delegation to RPNs and Personal Support Workers (PSWs)
Care coordination with physicians, hospitals, specialists, and community providers
Patient and caregiver education for complex medical conditions and treatments

Registered Practical Nurse (RPN) Services

Registered Practical Nurses provide nursing care for patients with stable or predictable health conditions, under RN oversight where required. Typical RPN services include:

Routine health monitoring and vital signs assessment
Medication administration for stable patients
Basic wound care and routine dressing changes
Injections and routine nursing treatments
Catheter care and ostomy maintenance

Catheter care and ostomy maintenance
Catheter care and ostomy maintenance
Post-procedure follow-up for stable clients
Patient education related to routine care and treatment plans
Monitoring and reporting changes in patient condition to the RN or physician
