Post-Discharge Health Assessments
Initial and follow-up assessments to evaluate recovery progress, identify risks, and ensure care needs are addressed after discharge.
Medication Reconciliation & Support
Review and organization of medications following hospital discharge to ensure accuracy, safe administration, and adherence to prescribed treatment plans.
Recovery Monitoring & Follow-Up Care
Ongoing monitoring of symptoms, vital signs, and overall health status to support recovery and identify concerns early.
Wound & Surgical Site Monitoring
Assessment and care of surgical sites or wounds related to recent procedures, including dressing changes and infection prevention as needed.
Care Coordination
Communication and coordination with physicians, hospitals, and other healthcare providers to support continuity of care and smooth recovery.
